Sixteenth Street Community Health Center

Sixteenth Street has provided high quality health care, health education and social services to low-income residents of Milwaukee’s culturally diverse south side since 1969. Comprehensive health services include adult and pediatric medicine, behavioral health services, women’s health, HIV prevention and treatment and physical and occupational therapy.

Our mission is to improve the health and wellbeing of Milwaukee and surrounding communities by providing quality, family-based health care, health education, and social services free from linguistic, cultural, and economic barriers.

Sixteenth Street is a Health Center Program grantee under 42 U.S.C. 254b and a Public Health Service employee under 42 U.S.C. 233(g)-(n). Additionally, Sixteenth Street has been accredited by the Joint Commission, meaning that our patient care and facilities meet national standards for quality and safety. Our accreditation was renewed on June 14, 2012 and is valid for up to 39 months following that date.

In April 2013, Sixteenth Street received recognition as a Level 3 Patient-Centered Medical Home by the National Committee for Quality Assurance.

Keeping with our tradition of emphasizing the important relationship between environment and the health of our patients and of the community, Sixteenth Street Community Health Centers have been awarded Bronze level recognition as a Bike Friendly Business from the League of American Bicyclists.
